Some duties may include: assessments of patients with heart failure and transplant-related conditions to determine appropriate diagnoses and treatment plans, effectively communicate diagnoses, treatment plans, and follow-up care requirements to patients and their families, and formal and informal education to staff on best practices in heart failure and transplant care.
