Salary: $135,200 - $156,000 / annually

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

Work Schedule:
This is a full-time, exempt position with a standard work schedule of Monday through Friday, 8:00 AM to 5:00 PM. While the role generally follows these hours, flexibility may be required to meet business needs, including occasional evenings or weekends.

Who You Are:

You are a motivated Physical Therapist with proven success in a supervisory role, who shares our vision of delivering in-home care. You treat patients in a caring and compassionate manner and help them achieve their goals of staying in their own home whilst receiving treatment.

Duties and Responsibilities:

To perform this job successfully, you must be able to perform the following duties successfully:

  • Plans, develops, organizes, implements, evaluates and directs the overall operation of the rehabilitation department as well as its programs and activities, in accordance with current state and federal laws and regulations; and respective practice act(s) in the state.

  • Identifies, in conjunction with the Operations Manager, the departments key performance indicators. Establish an ongoing system to monitor these key indicators such as the Quality Assurance and Performance Improvement process in the rehabilitation department.

  • Develops and implements processes and systems to effectively lead, guide and direct the delivery of rehabilitation services including but not limited to processes for the timely screening, evaluation, treatment, discharge and all associated documentation requirements for residents in the facility.

  • Ensures that all therapists are appropriately documenting, billing and coding for patients receiving therapy.

  • Serves as the companys rehabilitation representative and spokesperson to the public as well as in the professional arenas. In so doing, must periodically deliver a confident, concise, message on a variety of topics that is understood by a wide range of people with diverse backgrounds and levels of education.

  • Develops and maintains positive relationships with patients, employees, and family members through scheduled, deliberate positive interactions with them using face to face, phone, or other means of communication.

  • Identifies and collaborates with members of the interdisciplinary team, physicians, consultants, and community agencies to identify opportunities for enhanced services to the patients and/or resolve issues.

Essential Requirements:

  • A masters degree in physical therapy
  • Minimum of 3 years experience in a supervisory role as a home health care provider or similar role.
  • Current California State license as Physical Therapist
  • Current CPR certification
  • Valid California Drivers License
  • Eligible to be insured under our liability policy with a clean DMV report
  • Your own car, and proof of current auto insurance

  • Demonstrates proficiency in performing physical therapy procedures and modalities and in assessing the effects of these
  • Demonstrates sensitivity with patients, family, and their medical team
  • Ability to work with others, at all levels within the organization, and collaborate effectively.
  • Above-average interpersonal, problem-solving, and written and oral communication skills.
  • Professional behavior when on duty
  • Ability to maintain patient records and able to organize daily schedule

Physical Requirements:

  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours.
  • Able to lift 50 pounds without assistance.
  • Able to drive intermittently on a daily basis

Compensation and Benefits:

Pay range is between $135,200 to $156,000 per year. Exact compensation may vary based on skills, experience, and location.

  • You will be reimbursed for the mileage you travel from the office to participants homes.
  • You will receive a monthly phone stipend.
  • On the first of the month following 2 months of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, long term disability insurance as well as Flexible Spending Account (FSA).
  • We offer 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked

Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.
